UPLIFT Procedure: Cystic Masses and a Retroverted Uterus

Patient History
A 25-year-old woman, nulliparous, experienced severe pain with her periods. The pain started one day prior to
her periods and had been experienced for 10 years. In addition, she experienced significant pain with intercourse.
Pain with periods was rated a 9 with "0" being no pain at all and "10" the worst pain ever experienced. Pain with intercourse was rated an 8.
Previous treatments included oral contraceptives, non-steroidal anti-inflammatories and codeine.
Physical Examination
Cystic masses on the left side and a severely retroverted uterus were found upon physical examination. The pain experienced during
intercourse was reproduced with deep pressure on the cervix and uterus.
Diagnostic Laparoscopy
Laparoscopic examination revealed venous congestion, which was very notable on the left side, a 5 cm endometrioma on the left side,
endometriosis in the cul-de-sac and a retroverted uterus.
Dilated veins consistent with venous congestion.
Retroverted uterus with venous congestion.
Procedure
The endometrioma and endometriosis were resected. The UPLIFT procedure was performed to reposition the uterus to a neutral position.
Clinical Results
Pain with periods and with intercourse was greatly reduced. Pain with periods was rated 2 and pain during intercourse was rated a
1 on a 10 point scale. The results at 36-month follow-up have been consistent.
