Full-thickness Trocar Wound Closure with the Carter-Thomason CloseSure System XL
Designed especially for obese and bariatric patients
The Carter-Thomason CloseSure System XL features the Carter-Thomason® suture passer
and 10/12 mm and 15 mm Pilot® guides.
Use the 10/12 mm Pilot guide to perform full-thickness trocar wound closure in the
following manner:
Step 1

Insert the Pilot guide with the holes aligned cephalad to caudad.

Use the Carter-Thomason suture passer to push suture material through the Pilot guide, fascia, muscle, and peritoneum into the abdomen.

Drop the suture, and remove the suture passer.
Step 2

Push the suture passer through the opposite side of the Pilot guide, and pick up the suture.
Step 3

Pull the suture up through the peritoneum, muscle, fascia, and guide.
Step 4

Remove the Pilot guide and tie.
Use the 15 mm Pilot guide to perform full-thickness trocar wound closure in the following manner:
Step 1
- Insert the Pilot guide and position it with the "Head" label oriented towards the patient's head.
- Starting with the hole labeled 1, use the Carter-Thomason suture passer to push suture
material through the Pilot guide, fascia, muscle, and peritoneum into the abdomen.
- Drop the suture, and remove the suture passer.
Step 2
- Push the suture passer through the hole labeled 2, and pick up the suture.
Step 3
- Pull the suture up through the peritoneum, muscle, fascia, and guide.
Step 4
- Complete a second pass by repeating steps 1-3 using another piece of suture and the holes labeled 3 and 4.
Step 5

Remove the Pilot guide and tie both sutures to complete the closure.
