Reimbursement Guidelines for The ELEVEST^® Procedure

The ELEVEST procedure may be submitted for reimbursement under existing procedure codes.

1. Specific reimbursement levels will vary according to geography, insurance carrier and/or contracted fee.

4. Provide detailed documentation of all the steps necessary to complete the procedure. Multiple diagnoses and procedures codes (if applicable) may be necessary.
